A paraprofessional who is specifically trained, competent, and performs assigned functions of personal care to the patient in their residence under the direction, instruction, and supervision of the registered nurse (RN) or other qualified professional.

Additionally, the CNA will:

  • CNA/HHA performs personal hygiene and grooming tasks appropriately and safely that are

assigned by a Registered Nurse (RN) or other appropriate skilled professionals, includes bathing, hair care, nail care, skin care, oral hygiene, feeding, toileting, and elimination.

  • CNA/HHA maintains open communication process with the patient and/or representative (if any), caregivers, and family. Able to explain what he/she is going to do with the patient, ask the patient open-ended questions, seek feedback from the patient, and respond to the needs and requests of the patient, representative (if any), caregivers, and family.
  • CNA/HHA performs a normal range of motion and positioning.
  • CNA/HHA verbalizes understanding of adequate nutrition and fluid intake.
  • CNA/HHA demonstrates knowledge of basic elements of body functioning and changes in body function that must be reported to the RN or other appropriate skilled professionals.
  • CNA/HHA demonstrates knowledge of recognizing and reporting changes in skin condition to the RN or other appropriate skilled professionals.
  • CNA/HHA performs all aspects of patient care in an environment that optimizes patient safety.
  • CNA/HHA demonstrates the ability to work with the population served by the agency to their physical, emotional, and developmental needs including the need for respect for the patients rights, his or her privacy, and his or her property.
  • CNA/HHA verbalizes and demonstrates an understanding of being a part of the interdisciplinary team.
  • CNA/HHA demonstrates the ability to document appropriately in the patients medical record.
  • CNA/HHA completes at least 12 hours of in-service training during a 12-month period.
  • CNA/HHA participates in other agency mandated activities, i.e., in-services, staff meetings.


  • Help optimize the capital structure and support strategic initiatives.
  • One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
  • Maintains the agency’s mission, philosophy, and core values.
  • Adheres to dress code, appearance is neat and clean, wears identification while on duty.
  • Reports to work on time and/or as scheduled.
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
  • Always maintains patient confidentiality, including all HIPAA regulations.

Regulatory Requirements

  • Licensed or registered in the State of Michigan.
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).

Industry experience

  • Per CMS regulations, a qualified home health aide is a person who has completed:
  • A training and competency evaluation program that meets the requirements of 42 CFR §484.80(b) and §484.80(c)* respectively; or A training and competency evaluation program that meets the requirements of 42 CFR §484.80(b) and §484.80(c)* respectively;
  • A nurse aide training and competency evaluation program approved by the state as meeting the requirements of 42 CFR §484.151 through §483.154 and is currently listed in good standing on the state nurse aide registry;
  • The requirements of a state licensure program that meets the provisions of 42 CFR §484.80(b) and §484.80(c).
  • The requirements of a state licensure program that meets the provisions of 42 CFR §484.80(b) and §484.80(c).
  • A continuous period of 24 consecutive months during which none of the services furnished by the individual as described in 42 CFR §409.40 were for compensation.
  • If there has been a 24-month lapse in furnishing services for compensation, the individual must complete another training and competency evaluation program before providing services.
  • *Refer to the Home Health Aide Service Policy PC2.52 for requirements of 42 CFR §484.80(b) and the Certified Home Health Aide Competency Evaluation Program for requirements of 42 CFR §484.80(c).


  • Preferably a high school diploma or equivalent.
  • If working as a CNA certification is required.
